Patient Information

An otolaryngologist is a specially trained medical doctor who treats problems with the ear, nose and throat. These physicians are commonly known as "ENT" doctors. An otolaryngologist has completed a minimum of four years of specialized surgical training for the head and neck in addition to medical school.

Many Central Oregon Ear, Nose & Throat physicians have also completed advanced fellowship training in a subspecialty, such as surgical treatment of the ears, larynx, or sinuses.

Central Oregon Ear, Nose & Throat is the premiere otolaryngology practice in Central Oregon. All our physicians provide general otolaryngology services even if they practice in a subspecialty.

New patient forms (Adobe PDF) are available for you to download and complete prior to check-in at your first appointment. To expedite your check-in, mail them ahead or fax the forms to medical records at (541) 385-4935.

Emergencies

Please contact 9-1-1 or proceed to the hospital emergency room if your medical problem cannot wait for a return call or later appointment. Please note that insurance companies sometimes stipulate how emergency visits will be paid.

During business hours, call (541) 382-3100 and ask for the triage nurse.

After hours, the answering service will locate the physician on call.

Prescription refills

Contact your pharmacy directly for refills. Please contact the triage nurse at (541) 382-3100 for refill instructions if your refill requires a written prescription.

Sinus Patients

Patients seeking appointments with Dr. Gallivan at the Central Oregon Sinus Center should compete one of the two following forms:

Sinus Questionnaire for new patients.

The billing office is open 8 a.m. - 5 p.m. Monday through Friday. Payment is expected at the time services are received in the clinic. A deposit may be required for surgeries, procedures, or durable medical equipment. Please contact the billing office for the most current information.
